HOW DO I.... REGISTER WITH THE PRACTICE? Simply bring your medical card to the surgery together with proof of identity and address and, provided you live within our practice boundary, our receptionist will register you with the practice. If you cannot find your medical card, the receptionist will ask you to fill in an alternative form. You may express a preference to receive services, either generally or in relation to a specific condition, from any of the doctors or practice nurses. Such expression of preference must be made in advance of the relevant appointment(s). However, this may be dependent upon availability of appointment, the nature of the condition or the urgency of the problem, in which case you may be asked to see an alternative doctor or practice nurse. As we have two sites, you may be seen in either surgery. We will make the arrangements for transferring any information we may need for your consultation. |
